Office visits are by appointment only. Even for an emergency, we need to figure out when we can make time to tend to your pain properly. Please call us well in advance for your appointment and tell our staff in detail what you need.
If you are a new patient, please print out our new patient form and fill it out prior to your arrival to minimize your wait time.
Your account is to be settled in full each visit, unless prior arrangement is made in writing. If you have any questions or concerns regarding financial arrangement, please talk to one of our knowledgeable staff to discuss your needs.
Dental insurance, as with any kind of insurance, is a contract between you and your carrier. We will do our very best to make sure you receive all the benefits within your policy coverage. Please remember, the coverage varies from person to person and procedures not covered by your plan will be your responsibility.
It is always good to be actively involved in your claim. This office will fill out your claim form and submit it at no cost to you. Please realize that this service requires a considerable amount of resources on our end. Your cooperation in understanding your coverage is necessary and greatly appreciated.
Sometimes, it’s just not possible to make it. We understand those rare instances. Do call us if you need to reschedule as soon as possible. We require 48 hours of phone notice (not email or voice mail) to rearrange our schedule to accommodate your request. Kindly remember, all of our staff is at the ready for your scheduled time.

X-rays are the property of this office. Duplicate records are available at a nominal cost, upon written request. Please allow minimum of three business days for this service.
